CaCoon Care Coordination works with families with children birth through age-20 years with special health care needs.

What services do we offer?

A Public Health Nurse care coordinator with special training will help families learn to care for their child with special health care needs by-

  • identifying developmental and health problems early.
  • providing information about your child’s medical & special health care need.
  • teaching special techniques for caring for your child.
  • listening and giving support in times of stress and crisis.
  • finding and advocating for important services close to your home.

Families can have home visit services up to their child’s 21st birthday.
